Intake Form:

Please fill out the New Client Intake Form included in your appointment confirmation email at least 2 hours in advance of your session. Otherwise, you must arrive 10-15 minutes early to fill out the paper version at the office. 


Cancellation Policy:

Sometimes things happen and you have to cancel or reschedule your appointment and that's OK. There is no fee if notification is given at least 24 hours prior to your appointment.


Notification that is given less than 24 hours prior to the appointment time will result in a 50% charge of the reserved service amount.


Failure to arrive for your appointment will result in a 100% charge of the reserved service amount.



Arriving Late:

Out of respect and consideration of my time and other clients, please plan to be on time. Arrive 5-10 minutes early for any appointment in order to get the full time scheduled. If you arrive right on time or late, I can only give you whatever time remains of your appointment and you will be responsible to pay the full length of the session you requested.


*This time is scheduled just for you and only you which is why I require a credit card on file or prepayment to reserve and guarantee your appointment. However, it WILL NOT be charged unless you do not adhere to the Cancellation/No- Show policy.
